Drug Recall

 

HPS Pharmacies wish to advise that GlaxoSmithKline Australia, in consultation with the Therapeutic Goods Administration have given notice of an urgent drug recall of:

Zovirax® Ophthalmic Ointment

Presentation Aciclovir 3% (30mg/g) tube
Aust R 11013
Batch Number 3L942 4B909
Expiry Date Oct, 2015 Feb, 2016

 

This recall has been issued due to the potential contamination of these two batches of Zovirax® Ophthalmic Ointment with metal filaments. Filaments found in the raw materials range in size from 150 to 850µm, which is equivalent to the size of sand grains.

Due to these batches representing a significant proportion of stock available, and no other brand of aciclovir ophthalmic ointment being available in Australia, supplies are now constrained to meet the emergency needs of clients. Supply is not expected to return to normal until the second quarter of 2015.

No other batches of Zovirax® Ophthalmic Ointment, or other Zovirax® products, are affected by this recall.

Please inspect your stocks and quarantine all units for return to your supplier. Any items supplied by HPS Pharmacies should be returned to the pharmacy for processing.

Retain this notice in a prominent position, including in other related business units until normal supply resumes. Report any problems identified with medicines, vaccines or medical devices to the TGA.
