Drug Recall


HPS Pharmacies wish to advise that Apotex Australia has issued a drug recall of:

Midazolam Apotex Ampoules
Midazolam 5mg/5ml
ARTG 217663

The only batch affected by this recall is B5A0130 with an expiry date of April 2020.

This notice was issued following the discovery of compromised packaging on some ampoules from the above batch. Affected ampoules are not correctly sealed which poses a sterility issue and may lead to medication leaking from the glass ampoule.

If HPS Pharmacies provide imprest management services to your facility, an HPS representative will inspect your stock and quarantine all affected products. Your HPS Pharmacies Pharmacy Manager/Director of Pharmacy will contact your clinicians if there is an immediate impact to your facility.

Retain this notice in a prominent position, including in other related business units until supplies return to normal. Report any problems identified with medicines, vaccines or medical devices to the TGA.
