The notice has been issued because there has been one report of a syringe containing glass-like material blocking the action of the syringe so that the full dose could not be administered.
As there is no safety concern for patients and as this type of event is so unlikely (less than 1 in 10 million vaccines distributed), the product will not be recalled. Health professionals are reminded to visually inspect all pre-filled syringes prior to administration according to best practice for all injections.

Any problems identified with medicines, vaccines or medical devices should be reported to the TGA.
Please retain this notice in a prominent position, including other related business units, for one month as a reminder for all staff members.
