HPS Pharmacies wish to advise that AFT Pharmaceuticals, in consultation with the Therapeutic Goods Administration (TGA), has given notice of a drug recall of:
Lax-Sachets® 13.125g Sachets
Macrogol 3350 & Electrolytes
The only affected batches are as follows: 7229215, 7229216, 7229217, and 7229219.
This recall has been initiated after identification of an error in the dosage/administration instructions printed on the back of sachets from the affected batches. The incorrect directions on affected sachets read: “Each sachet should be dissolved in 1 litre of water.” The correct instructions should read: “Each sachet should be dissolved in 125ml water. For use in faecal impaction 8 sachets may be dissolved in 1 litre of water.” Following the incorrect instructions will result in a larger volume and more dilute final solution. Although unlikely to cause patient harm, effectiveness of the product may be reduced. The correct information is provided on the product carton and package insert.
If HPS Pharmacies provide imprest management services to your facility, an HPS representative will inspect your stock and quarantine all affected products for correction. Your HPS Pharmacies Pharmacy Manager/Director of Pharmacy will contact your clinicians if there is an immediate impact to your facility.
Retain this notice in a prominent position, including in other related business units for one month. Report any problems identified with medicines, vaccines or medical devices to the TGA.
