Drug Recall


HPS Pharmacies wish to advise that Alphapharm have given notice of a voluntary
drug recall of:

Ceftriaxone Alphapharm® Powder for Injection

Contents ceftriaxone (as sodium), 1g
Aust R 164920
Batch No. A3003
Expiry Dates 12/2014


This recall has been issued because of a risk of contamination of the vials. A foreign object found in one vial was part of a plastic cable tie introduced during maintenance.

This batch represent all stocks of Ceftriaxone Alphapharm® 1g Powder for Injection currently held by Alphapharm, and it is recommended to use alternative brands until supply returns to normal. Supplies distributed previously from other batches are not affected.

Please inspect your stocks and quarantine all units from the batch specified above for return to your supplier who will arrange for refund or replacement. Any items from this batch supplied by HPS Pharmacies should be returned to the pharmacy for processing.

Retain this notice in a prominent position, including other related business units for one month in case stock is in transit. Report any problems identified with medicines, vaccines or medical devices to the TGA.
